Mickleover Medical Centre

HOW DO I...
MAKE AN APPOINTMENT?

Our appointment system is designed to offer all patients access to a healthcare professional within 24 hours and a GP within 48 hours, regardless of whether or not the problem is urgent. However, in order for us to do this, you may occasionally be asked to see ANY doctor or nurse, as opposed to the one of your choice.

HOW DO I...
OBTAIN TELEPHONE ADVICE?

If you need to speak to one of our clinicians over the telephone during normal working hours, please contact our reception staff who will be happy to advise you as to how to proceed.

HOW DO I...
OBTAIN A HOME VISIT?

(01332 518101) (After 9.30am ring 01332 513283)

Home visits are only for patients who are too ill or infirm to attend the practice. Please ring us before 9.30am and give as much information as possible to the Triage Nurse in order that your request can be given due priority.

HOW DO I...
OBTAIN MY TEST RESULTS?

Tel: 01332 513283. Please ring after midday Monday to Friday when the telephone is not so busy and the reception staff have more time to find your results. Please allow at least one week for blood tests, two weeks for x-rays and four weeks for cervical smears. Please note, results will only be discussed with the patient or carer/guardian to ensure confidentiality.

HOW DO I...
REGISTER WITH OUR PRACTICE?

Assuming you reside within our practice boundary, we are happy to accept you as a new patient. In order to ensure this is the case, please check with our reception staff. On registering with the practice, you will be required to complete a form GMS1 for each member of the household. All members of the household of five years and over will then be asked to make an appointment for a New Patient Health Screen with one of our practice nurses, where routine checks on matters such as height, weight, blood pressure, smoking status etc will be undertaken. Please also bring a urine sample and any current medication with you when you attend this appointment.

It would also be helpful if you could provide us with any immunisation history for all children under five years.
