HOW DO I.... Please do not forget to inform the practice should you change your address. You will be given a form to complete and a member of the reception team will be able to advise you whether or not you are still within the practice area. It would also be helpful if you could let us know of any change in name or telephone number too, particularly ex-directory numbers. |
